Will I always see the
same therapist?
Yes,
once you are scheduled to see a particular PT,
you will continue to work with that person. However,
there may be exceptions due to unforeseen circumstances.

should I wear for the session?
No
matter what you are being seen for, please bring
a pair of shorts and a T-shirt for your treatment
sessions. We have showers and lockers available.

| How
long does each session last and how many will
I have?
Most
sessions last 45 to 90 minutes. Your PT will prescribe
your treatment and advise you as to the length
of each session. Your actual number of sessions
will depend on your specific condition and healing
time.

do I schedule appointments?
During
your initial evaluation, your physical therapist
will discuss with you the length and number of
treatment sessions recommended to meet your goals.
All appointments must be made through our front
desk staff. We strongly suggest you schedule your
appointments several weeks in advance, so that
we can better accommodate your schedule.

if I have to cancel an appointment?
Our
policy requires that you call 24 hours in advance
to cancel an appointment. Please call our
office and leave a message on our voice mail if
you need to reach us after hours. This
way we can meet the needs of other patients who
need appointment times as well.

will I pay for my physical therapy? |
We
are participating providers for the following

Healthcare
Please
call for additional plans

We require
that you pay your co-pay or co-insurance amount
at the time of service. We will bill your insurance
carrier for your physical therapy treatment.
We cannot
guarantee that the information we receive from
your insurance carrier is correct. Therefore,
you may be responsible for any deductible or balance
not paid by your insurance company.
